  1. We have a full time position (9am-5:30pm) available at our facility in Englewood, Ohio (Cypress Pointe Health Campus). This position would be 5 days/week with an every other weekend rotation and is located in our Memory Care unit.

 

  1. We also have two part time positions available at our facility in Zanesville, Ohio. (The Oaks at Northpointe). This position would be 2-3 days/week with an every other weekend rotation and is also located in our Memory Care unit.

 

  1. Highland Oaks Health Center has a part time position available in McConnelsville, Ohio. This position would be 2-3 days/week with an every other weekend rotation and is also located in our Memory Care unit. The position is only 4 hours/shift.

All positions within memory care help with our Daily Rhythms Programming. This keeps our residents on a schedule of activities including Morning Refresher Exercises, Cooking Classes, Crafts, Music/Singing, Trivia, etc.

 General Duties and Responsibilities:

This position assumes the primary responsibility for planning and implementation of a comprehensive, creative and innovative activity program designed to encourage optimal level of function for residents. 

Duties:

·         Plans, organizes and implements therapeutic group and individualized activities for the specialized units.

·         Writes a printed calendar of activities for the assigned unit.

·         Transports residents to and from activities and outings.

·         Maintains bulletin boards and holiday decorations.

·         Monitors, cleans, maintains and purchases activities equipment.

·         Attends weekly staff meetings to review and organize areas of responsibility and to review residents’ condition.

·         Plans, supervises and coordinates the orientation of visiting students during their affiliations. (i.e. OT & recreational)

·         Attends plan of care meetings and observes, records and reports on residents condition.

·         Completes in a timely manner all paperwork, documentation and assessments related to the plan of care and MDS 2.

·         Prepares physical arrangement schedule for activities, including set up and food requests.

·         Responsible for staffing some evening, weekend and holiday activities.

Educational Requirements:

  • Completion of the Basic Activity Course required.
  • Must be a qualified activity professional based on state and/or federal guidelines
  • Must be able to deal tactfully with Residents, Family Members, Staff Members and the general public.
  • Must have knowledge of and be willing to work with the aged and chronically ill who are mentally, cognitively, and/or physically impaired.
  • Must be physically capable of assisting residents with their personal needs including ambulating and/or moving residents throughout the facility.

 General Duties and Responsibilities:

 The Adult Day Program Coordinator will oversee the planning and implementation of daily activities and therapeutic groups for participants, assists with maintaining electronic database statistics and related program records and reporting. Supervises activities assistant(s) and is a member of the interdisciplinary team formulating participants’ care plans documenting level of functioning and progress towards established goals.

Educational Requirements:

  •  High school diploma or equivalent.
  • Bachelor’s Degree in Recreational Therapy or related field OR
  • Two years’ experience as Activity Coordinator or Director or related position; and/or demonstrates completion of National Certification Council of Activities Professionals.
  • 2 years’ experience working with older adults with physical and mental impairments.
  • Experience and aptitude for working with computerized systems including Word and Excel.
  • Excellent verbal and written communication skills.
  • Attention to detail and accuracy are essential.
  • Ability to prioritize work and consistently meet deadlines, and work as a team player with all departments.
  • Drug Test and Background Check required.
